This form constitutes a formal request under the Promotion of Access to Information Act, 2 of 2000 (PAIA). The practice is required to respond within 30 days of receipt.
Practices may charge a fee of up to R35 per page for providing copies of records (PAIA Schedule 2, Government Gazette No. 23119). You will be notified of any applicable fee before records are released.
If the practice does not respond or refuses your request without valid grounds, you may escalate to the South African Information Regulator.
